About the event

Do primary care clinicians have the resources (time, community contacts, etc.) to support the patient in adopting healthy lifestyle behaviors? Are patients and employees well-equipped to make lasting changes that can impact their health before turning to medications and treatments that are so often at their easy disposal? So much of chronic care management is dependent upon nutrition, physical activity, mental well-being, and other lifestyle factors. How can primary care clinicians be better supported to help patients fight chronic disease and lead healthier lives?

These questions are key to understanding how primary care clinicians serve as a driver of lifestyle behavior change and how employers can partner with them to increase employee buy-in and motivation to use lifestyle as a first line of defense for chronic disease. On December 4th, Community Connections will explore this topic as Dr. Padmaja Patel, President of the American College of Lifestyle Medicine and longtime primary care physician, shares her experience integrating lifestyle medicine into value-based primary care. Join us as we uncover how physicians, employers, health systems and health plans can work together to advance support for more successful and widespread lifestyle medicine in primary care!

Community Connections is an opportunity for primary care clincians, health care purchasers, health system and other stakeholder leaders to share experiences, perspectives, and ideas for strengthening primary care.
